Advanced Age and Fertility: Your Options After 35

The conversation around age and fertility can feel intimidating, but knowledge is power. Understanding how age affects your reproductive health helps you make informed choices — whether you're planning now or for the future.

    How Age Affects Female Fertility

Women are born with all the eggs they will ever have — roughly 1-2 million at birth. By puberty, this drops to about 300,000-400,000. By age 35, both the number and quality of eggs decline more rapidly.

    Key milestones:

    • Under 30: Peak fertility, about 25-30% chance of conceiving each month
    • 30-35: Slight decline, about 20% chance per month
    • 35-37: More noticeable decline, about 15% per month
    • 38-40: Significant decline, about 10% per month
    • Over 40: About 5% per month naturally; higher rates with IVF

    Why Egg Quality Matters

As eggs age, chromosomal abnormalities become more common. This is why:

    • Miscarriage rates increase after 35
    • Down syndrome risk increases with maternal age
    • IVF success rates decline with age

    Your Options

Natural conception: Many women conceive naturally after 35. Don't assume you need treatment without trying first (unless there are known issues).

Fertility testing: AMH (Anti-Müllerian Hormone) and antral follicle count can assess your ovarian reserve. Knowledge helps you plan.

Egg freezing: If you're not ready for children, freezing eggs at a younger age preserves their quality for future use.

IVF with your own eggs: Still effective in the late 30s and early 40s, with success rates declining but remaining viable.

IVF with donor eggs: For women over 42-43 or with diminished ovarian reserve, donor eggs offer success rates of 50-60% per cycle regardless of the recipient's age.

PGT-A (Preimplantation Genetic Testing): Testing embryos for chromosomal normality before transfer can improve success rates and reduce miscarriage risk in older women.

    Optimising Your Chances

    1. Don't delay testing: If you're over 35 and haven't conceived after 6 months, seek evaluation
    2. Optimise your health: Nutrition, exercise, sleep, and stress management all matter
    3. Consider supplements: CoQ10, DHEA (under medical supervision), and vitamin D may support egg quality
    4. Reduce toxin exposure: Limit alcohol, quit smoking, reduce environmental toxins

    Male Age Matters Too

While less dramatic, male fertility also declines with age. Sperm quality decreases after 40, with increased risks of genetic abnormalities.

    A Message of Hope

We see patients in their late 30s and 40s achieve successful pregnancies regularly. While the statistics can seem daunting, remember that you are not a statistic — you are an individual, and your fertility potential is unique.

The most important step you can take is to get assessed. Knowledge removes uncertainty and opens the door to action.
